Cannabis compounds CBD and CBG may help reverse fatty liver disease

Scientists have identified two non‑intoxicating cannabis compounds—CBD (cannabidiol) and CBG (cannabigerol)—that may reduce liver fat and improve metabolic health. These compounds appear to support liver function in two key ways: they enhance the liver’s ability to store backup energy and restore cellular systems responsible for breaking down harmful waste. Together, these effects suggest a potential plant‑based approach to treating fatty liver disease.

The findings indicate that CBD and CBG may improve liver health by altering how liver cells manage energy and clear unwanted material.

Metabolic dysfunction–associated steatotic liver disease (MASLD), the most common chronic liver condition worldwide, affects about one-third of adults. It is strongly linked to obesity, hypertension, and insulin resistance. Current treatment relies mainly on lifestyle changes such as diet and exercise, which can be difficult to sustain, and there are few approved medications—prompting the search for new therapies.

The researchers found that CBD and CBG do more than reduce fat accumulation. They induce “metabolic remodeling,” improving how liver cells function internally. Notably, both compounds increased levels of phosphocreatine, an emergency energy reserve that helps liver cells maintain stability under metabolic stress, such as a high-fat diet.

The study also showed that CBD and CBG reactivate cathepsins—enzymes within lysosomes that act as the cell’s recycling system. Restoring this process enhances the liver’s ability to break down and remove harmful fats and waste. As a result, levels of damaging lipids, including triglycerides and ceramides (which are linked to insulin resistance and inflammation), were significantly reduced.

While both compounds produced beneficial effects, they differed slightly in their impact. Both improved blood sugar control, but CBG showed stronger effects overall—reducing body fat, increasing insulin sensitivity, and lowering total and LDL (“bad”) cholesterol more than CBD.

Although these findings are promising, further research is needed to determine whether these effects translate to humans. Still, the study highlights a novel therapeutic direction: targeting cellular energy storage and waste clearance using plant-derived compounds to treat fatty liver disease and related metabolic disorders.
